1. What has been done in reality to reduce the risk of another event similar to Macondo/Deepwater Horizon?

The causes of the tragic disaster in the Gulf of Mexico in 2010 are well documented.

However, actions taken to prevent a recurrence are hard to find.

What has been done to, for example:

1. Improve decision making with respect to:

• Choice of cement for well sealing
• Improve assessment of specific technical tests such as those known as ‘full returns’  and ‘well integrity test’
• Ensure that the blow-out preventer is understood to be effective only in the first stages of loss of well control, and not once a high rate of loss of well fluids is occurring.
• Effective and practical emergency control plans for loss of control of the well.
